Description The second phase of our research project focused on determining the conditions for inclusive education at primary schools. Our attention focused on how is inclusion in school perceived and evaluated by teachers. Experience with the implementation of self-assessment survey instrument "Index for Inclusion" were analyzed and simplified version of this tool proposed for implementation in Czech primary schools.
